Counseling Availability

Before scheduling an appointment, I offer a free 15 minute consultation to obtain relevant information and determine fit.

Do you take insurance?

I am an in-network provider for Blue Cross Blue Shield and Aetna.

What if I have a different insurance plan?

I am an out of network provider for most other PPO/POS insurance plans. This means that if your plan includes out-of-network benefits, you can collect partial reimbursement from your insurance company for the cost of sessions. (PPO/POS plans often reimburse therapy clients for 60-80% of session costs. Every plan is different, so please check with your insurance provider to determine your out-of-network arrangement.) Please note, I cannot guarantee you will receive reimbursements as all interactions with your insurance company are your responsibility.

Ways to contact me

You can always fill out the form in the "contact me" section, but otherwise, feel free to contact me at aj@chosenheartcounseling.com

When contacting your insurance company it can be helpful to ask:

Do I have out of network benefits for outpatient or behavioral health services What are the benefits? Do I need a referral or pre-authorization to use my out-of-network benefits? What percentage of the service fee is covered? How do I submit receipts/superbills for reimbursement? (Most insurance companies allow for invoices to be submitted via their website.)

My out of pocket fees are:

Initial Individual Intake Assessment - $275 Individual [Weekly] Therapy sessions - $225

Who is this practice a good fit for?

I work with clients who are ready to learn more about themselves, move through the world in their authentic self, prepared to pursue their goals in therapy and out in the world.

Who may need a different level of care?

Though I can match many client’s reasonable expectations of what to receive in therapy, I’m not going to meet what everyone is looking for in a therapist and that’s okay. Due to certain life factors I experience, my work with clients is not in a capacity to meet with clients who require consistent immediate care or more intensive care than weekly outpatient therapy and clients with recent (within the past year) inpatient care.

How often do clients meet?

I meet with clients on a regular weekly basis, with some flexibility on rescheduling within the same week.

Do you offer LGBTQ+ affirming therapy?

My clinical focus is on LGBTQ+ related issues, life transitions, and anxiety and depression.

What kinds of therapy do you use?

I use different counseling modalities based on clients' presented needs such as evidence-based approach Cognitive Behavioral Therapy (CBT), Psychodynamic, Person Centered, Multicultural, Solution Focused and Strength-Based Therapy.
